Transform™ Survey Hosting

No need to retype and reformat — you input participants' names and email addresses, or send out a link using your own email system. For other survey delivery options, see Transform Delivery Options. Transform collects the data and calculates raw scale scores by participant. You can monitor activity, send reminders, and download your data (.csv file) via your Transform Portfolio.

Terms of Use for Transform Survey Hosting (Data)
Transform Survey Hosting is a data license for research purposes only. This license grants you permission to collect and disclose (a) item scores and scale scores, (b) statistical analyses of those scores (such as group average, group standard deviation, T-scores, etc.) and (c) pre-authorized sample items only, as provided by Mind Garden, for results write-up and publication.

For example, with purchase of the Transform Survey Hosting (Data) and when presenting your findings:

  • You may share the group's mean scale scores with survey participants and others.
  • You may not share item text in publications with the exception of the pre-authorized sample items included with license purchase.
  • You may not copy, modify, or paraphrase content from Mind Garden Individual Reports and release that content to your survey participants or others.

Note: this list illustrates some permitted and prohibited uses of the instrument and is not meant to be all-encompassing.

If you are using this instrument for developmental purposes, you need to purchase online reports from Mind Garden (purchase of Individual Reports includes Transform Survey Hosting license).

The instrument items, directions, manual, individual report, group report, and any other descriptive information available through Mind Garden is the intellectual property of the copyright holder and can be used only with purchase or written permission from Mind Garden.

Translations
You can select and change your default language in your campaign (Campaign Details tab). Participants will see the default language first but can change the language while taking the survey.

Note: We cannot assure translation quality — many are made by individual researchers and we are not necessarily familiar with the particular language or dialect. Some of the translations are partial and typically do not have validation data. Basically, we offer whatever is available to facilitate your work.

Services and Reports

  • Group Reports calculate and summarize average scores for a group of participants and are available for purchase as an accessory.
  • Purchase MLQII 360 Leader's Reports to provide each participant with an interpretive report of his/her scores. Each report includes one (1) Transform Online Survey License.

Alternative License Types

  • Purchase MLQ License to Administer to administer the MLQ as an online survey via a non-Mind Garden survey system or as a paper/pen survey.

Bruce J. Avolio, Ph.D., is a Professor of Management at the University of Washington Foster School of Business where he also serves as the Executive Director of the Foster Center for Leadership. His specialties include Leadership Assessment and Designing Micro Development Interventions.

Bass, Bernard M.

Bernard M. Bass, Ph.D. (d. 2007) was a professor emeritus in the School of Management at Binghamton University (State University of New York). He was also the founding director of the Center for Leadership Studies at Binghamton and founding editor of The Leadership Quarterly journal.
