Ordering Details
There are four ways to order from Mindgarden:
- Online - Order online simply by clicking on the appropriate icon--for either pdf
or paper
format--
next to every product. The product icons indicate in which format you can buy an instrument. When you are ready to checkout, click on "checkout" at
the shopping cart page, and our secure commerce site
will process your order with your credit card (MasterCard or Visa).
- Phone - Phone us at (650) 322-6300. Please have a valid credit card (MasterCard, Visa, American Express, Discover) or a corporate purchase order number ready when you call. If you reach our voicemail system, leave a number where we can call you back and tell us the best time to try to reach you.
- Order Forms by fax or mail. Printable copies of
the Mind Garden order form are available here in pdf or MSDoc
format (forms open in new window). If
ordering by purchase order, please include your institution name and
purchase order number, as well as billing address. If sending a check,
your product will be shipped upon receipt of your check. You can fax

Payment Options
Mind Garden accepts payment by MasterCard, Visa, American Express or Discover cards, and accepts institutional purchase orders. Payments must be in U.S. funds. Checks must be drawn on funds from a U.S. bank.
Returns and Exchanges
- Reproduction sets and pdf files may not be returned.
- Returns will be accepted within thirty days of purchase.
- Returns must be sent to Mind Garden by certified mail or other traceable method.
- To receive credit, products must be in re-salable condition and accompanied by a copy of the original invoice.
- Shipping charges are nonrefundable.
Save time and avoid shipping charges
Regardless of whether you order the paper version or the pdf (electronic) version of an instrument, you will receive one copy of the instrument or document with license to reproduce the purchased quantity. However, by ordering the pdf/electronic version you will save time and avoid shipping charges.
PDF Delivery Time. Shortly after you order the pdf version and complete payment on your order, you will receive an email with instructions on how to retrieve the document you have purchased. If by some chance you do not receive that email, first check your Spam and Junkmail folders for an email from Mind Garden. If the email is not there, go to our home page and at the top, click on Transform. In the green box enter your "ship to" email address twice and create a password. Then you can view and print the PDF(s) you purchased. Assuming there are no technical problems on your end or ours, most PDFs will be available to you same day (Not instantly!). You are always welcome to email us by clicking here.
Expiration of PDFs. Your pdf will remain on our server for approximately one year.
About PDF Files / "No Returns Policy"
Many/most of our instruments are available for electronic purchase in
PDF
format. PDF files can be read with the free Acrobat reader, available
at Adobe. When a product is available in PDF format, you will see the
Acrobat icon
to the right of the product
title. It is important to know that this is exactly the document you want
because you cannot "return" a pdf document.
We'll say that again: Please be sure you've chosen the right document, because PDF purchases cannot be returned. If you have any questions about the preview procedure, please call or email Mind Garden.
Copyright and Translation Permission
Mind Garden policies about permission to reproduce in publications and other issues related to copyright are listed on the copyright page.
If you want to modify or translate an instrument, please contact us for a translation agreement. It is important that you sign a translation agreement before doing the translation. You must order the minimum reproduction licenses (50 licenses) before we will generate a translation agreement.

Statistical Support
Mind Garden recommends that you purchase a manual for the instrument with which you are conducting research. The manual will assist in administration, scoring, interpretation, and normative comparisons, all of which are necessary for appropriate use of the instrument. Should you need additional statistical support, one source we recommend is Statistics Solutions, Inc. Statistics Solutions consists of a team of Ph.D. research methodologists and statisticians specializing in psychological survey instruments, research design, data analysis, sample size, conducting the appropriate statistics and formulating the results. Thesis or Dissertation Appendices
With the purchase of reproduction licenses, you will receive a "5-sample item" letter. The letter will be the last page of your reproduction license document. For more information go to our copyright page.
Limitation of Liability
Mind Garden, Inc. will not be responsible or liable for the use or misuse of the materials or services offered in our catalog or website. The purchaser/user assumes all responsibility for use or misuse of the same. Mind Garden, Inc. will not be liable for any failure or delay in performance due in whole or in part to any cause beyond its reasonable control.
Mind Garden, Inc. makes no other warranties, express or implied, including warranties of merchantability or fitness for a particular purpose. In no event will Mind Garden, Inc., nor anyone else who has been involved in the creation or production of any of the printed works or software listed in this catalog, website or otherwise sold by Mind Garden, Inc., be liable for any indirect, special, or consequential damages arising out of the purchase or use of the materials or services from this catalog or website, notwithstanding any other provision or agreement between customer and Mind Garden, Inc. The customer's sole and exclusive remedy shall be correction or replacement of the printed work or of the software.
Putting an instrument online using an outside data collection website for your survey
Mind Garden provides Online Survey and reports for a number of web-based instruments. Full multi-rater data collection is available for the MLQ. Not all of our
instruments can be put on the Web (The drop-down menu
on our online agreement form contains a complete list of instruments that can be put online). PLEASE NOTE THAT DISTRIBUTING AN ENTIRE INSTRUMENT IN EITHER THE TEXT OF AN EMAIL OR AS AN EMAIL ATTACHMENT IS PROHIBITED.
If you would like to request permission to put one of our instruments into a Web-based form for data collection for your research (only data collection, no reports) by using an outside survey company such as Survey Monkey, we require the completion of the Online Use application (See link below). A minimum purchase of 50 reproduction licenses is required.
- The name of the psychological instrument
- The name of your research project
- Your name and institution
- A statement that you have compensated Mind Garden for at least 50 reproductions and will compensate Mind Garden for every administration of the online form
- A statement that you will put the instrument copyright on every page containing question items from this instrument
- A statement that you will remove this form from the internet/survey site at the conclusion of your data collection
- This form must not be on the "open web". You must require a unique login and password for every respondent (Survey Monkey calls this the Email Invitation Collector). State how you are putting the form up that requires a login. It is important to specify the data-collection service that you are using (e.g., university intranet tool, Survey Monkey, PsychData, etc.). IMPORTANT NOTE:You cannot send Mind Garden instruments in the text of an email or as a pdf file to participants.

Privacy and Data Policy
Data Policy: Mind Garden, Inc. (MGI) reserves the right to aggregate and use, without compensation to anyone, data obtained from the scoring or other processing of products published by MGI or otherwise sold or licensed by MGI, and to utilize those data for research, product development, and statistical or other purposes. MGI reserves the right to disclose such non-person-specific data to third parties, with or without compensation to MGI, as MGI sees fit from time to time.
Collection of Information on the Internet: Our web site may log a browser type and version when he/she visits the site. We use this information to help to administer our website. Our web site uses an order form for customers to purchase products and services. We collect customers' contact information (such as their address and email address) and financial information (such as their account or credit card numbers). We verify our customer-provided credit card information with a third party prior to order processing. Contact information from the order form is used to send orders to our customers. We also provide mailings and other marketing collateral regarding product information and releases.
MGI collects the name and e-mail address of the end-user (the assessment taker) for the sole purpose of providing MGI's customers with assessment results. MGI does not share this information with any third parties (other than the test administrator). Certain federal, state, local or other government regulations may require that we disclose personally identifiable information. In such cases, we will use reasonable efforts to disclose only the information required under applicable law.
Our web site may contain links to other web sites. We have no control over the privacy practices of such other web sites nor should the link be considered an endorsement of such other web sites' privacy practices. We recommend that you read the privacy policies of such other websites.
Security: When a user pays for an instrument or places an order online, the user's personal information and credit card information are processed and encrypted by offsite, secure servers using industry-standard SSL encryption. SSL is short for Secure Sockets Layer, a protocol developed by Netscape for transmitting private documents via the Internet. We follow generally accepted industry standards to protect the personal information submitted to us, both during transmission and once we receive it. No method of transmission over the Internet, or method of electronic storage, is 100% secure, however. Therefore, while we strive to use commercially acceptable means to protect your personal information, we cannot guarantee its absolute security. Will MGI Disclose the Information It Collects to Outside Parties? We do not sell, trade or rent or otherwise give to any other entity or organization the individual information our customers submit when placing an order or taking an instrument on our site except as noted in this privacy policy. MGI does provide its customers with the individual information they need to properly counsel or advise their clients. In other words if you are taking an instrument on our site at the direction of a consultant, counselor or other individual, that consultant, counselor or other individual will receive, from MGI, individually identifiable information so that he or she may properly counsel or advise you.
Does MGI Disclose Rater Identity in Multirater or Observer Ratings? MGI cannot protect confidentiality in the event where only one rater at a given level is entered and responds. MGI reports do not contain rater names. MGI observer data for research contains only the name of the entity being rated and not the names of individual raters.
